Caseworking is the process of helping patients connect with resources that address needs affecting their health and well-being. PHAs accompany physicians in clinic and, at the end of each visit, speak directly with patients to understand their needs. Within two weeks of the appointment, volunteers email the patient a personalized list of personalized resources and services. For example, if a patient has difficulty getting to the hospital, a caseworker might identify low-cost shuttle programs or other local transportation services that can help them attend future appointments.
