Transitional Care Project Team
The Transitional Care (TC) Team develops and promotes health care literacy for youth, adolescents, and college students to transition from pediatric care to adult care. The team works with UCLA pediatric physicians, residents, and medical students to create and maintain a clinical health care system for adolescents to begin managing their own health. Last year, TC focused on constructing resources to inform young adults on how to navigate the new responsibilities of adulthood, including employment, dental care, and overall healthcare. Our focus for the upcoming year is more direct youth outreach through partnerships with teenage youth organizations and local colleges to encourage greater independence in health settings and insurance navigation. This includes reaching out to high schools, youth clubs, and our fellow Bruins through tabling, presentations, and more!
Recent Initiatives

TC Dental Resources Referral Packet
The doctors and residents at the clinic noticed that many of their patients were looking for dental clinics that offered treatments for patients with special needs and specific fears/anxieties. The Transitional Care Team worked on creating a digital dental resources referral list with all necessary information & details like location, contact info, cost, treatment description, customer review, etc. so that the clinic staff can easily pull out the list to refer their patients.
