Transitional Care Project Team
The Transitional Care (TC) Team develops and promotes health care literacy for youth, adolescents, and college students to transition from pediatric care to adult care. The team works with UCLA pediatric physicians, residents, and medical students to create and maintain a clinical health care system for adolescents to begin managing their own health. Last year, TC worked with our advisors to develop transition resources for specialty care teams to share with patients and families. We also collaborated with external organizations to help young people learn how to maintain their health and take a more active role in their care. Our focus for the upcoming year is on understanding how LA County’s Medi-Cal and California Children’s Services (CCS) policies affect the transition to adult care, and developing resource guides to help young people and their families prepare for changes in coverage and care.
Recent Initiatives
Tabling at CAHPSA/SWC Health Fair with Outreach
This spring, TC collaborated with outreach to table at the CAHPSA/SWC Health fair, educating students in attendance on transitional care. TC handed out fliers with information on managing medical care and hosted an interactive activity for students to test their healthcare knowledge!
Cystic Fibrosis and Primary Care Partnership Pamphlet
Last fall, the TC team created a pamphlet for young adults with cystic fibrosis explaining why primary care remains important alongside their CF care. It outlines how primary care doctors and CF specialists work together and highlights routine care, including screenings, vaccinations, and mental health support.
